Dear New Patient,

PLEASE BRING A VALID PHOTO ID AND YOUR HEALTH INSURANCE CARD, IF ANY. WE NEED THIS INFORMATION TO CHECK YOUR HEALTH INSURANCE COVERAGE, IF THAT IS THE CASE.

We are sending you a medical questionnaire to complete at home. Please bring the completed packet to your appointment.

PLEASE BRING ALL YOUR CURRENT MEDICATIONS WITH YOU TO YOUR APPOINTMENT (including vitamins and supplements).

It is very important that we have pertinent records from your referring physician including laboratory reports, EKGs, treadmill tests, Holter monitors, or echocardiogram copies. You may bring copies with you or request that they be forwarded directly to this office. (See Authorization to Release Records form.)

We do insurance billing. We are happy to bill other group insurance plans provided the visit is paid at the time of service and current billing information is supplied. Please bring your physical insurance cards (no electronic-only cards). Deductibles and copayments are due at the time of service according to your plan.

Our schedule is heavily booked. Please arrive 15 minutes prior to your appointment with all forms completed. Notify us right away if you are unable to keep this appointment.

FINANCIAL POLICY

  • We bill Medicare, PPO, and Managed Care plans.
  • Deductibles and co-payments are due at time of service.
  • For non-contracted plans, payment is due at visit and we will submit claims on your behalf.
  • Please bring physical insurance cards (no electronic-only cards).
  • Returned checks are subject to a processing fee.
  • Missed appointments without notice may be subject to a fee.

Dr. Aanita Weber

Our Founder & Lead Developer
Dr. Aanita Weber, MD, is the founder and lead developer of Nasal Bioactive. In her late twenties, she combines a deep clinical understanding with a passion for translating scientific insight into innovative wellness solutions
After completing her medical degree at Heidelberg University she trained in medicine with a specialization in otolaryngology, Dr. Weber has dedicated her early career to exploring how targeted interventions in the nasal and sinus passages can enhance human health and sensory experience.
Since founding Nasal Bioactive, she has led the development of advanced intranasal formulations and experimental bioactive compounds, combining rigorous clinical methodology with inventive product design. Her work focuses on translating scientific insight into safe, effective, and forward-thinking solutions for everyday wellness.
Outside her professional work, Dr. Weber engages with emerging wellness technologies, enjoys exploring sensory experiences, and advocates for science communication that makes complex medical concepts accessible.
